Insurance & Referrals
As a courtesy, we file all insurances listed below. Please make sure all of your child’s information is accurate and up to date with the insurance company prior to their appointment. This includes: Primary Care Physician assignment, date of birth, spelling of name, coordination of benefits, and eligibility. If this is not done prior to the appointment, you may be asked to reschedule. Failure to do so will result if your child’s claim being denied and the total amount of the visit will become your responsibility.
*Please note, not all of our doctors are accepting all of the insurances below. Please call if you have any questions and we can direct you to the doctor that is accepting your insurance.

If you have any questions for the billing department, please call (850) 877-1162 and select the option for “billing”. As your Patient Centered Medical Home, we are here to help!
Referrals are processed in the order of medical necessity and as timely as possible considering the requirements of your insurance company. We ask that if your child has a test, procedure or appointment that you call our office well in advance to provide sufficient time to process any required referrals or authorization. Referrals generally require 3-5 business days to complete. If your child’s insurance requires an authorization, it may take 7-10 business days to obtain.
Please note, all information and reminders regarding your child’s referral will be sent via Patient Portal. If you do not have access to the Patient Portal, please call our office so we can assist in setting it up.
The preferred method of communication with our Referral Coordinator is through the Patient Portal. To expedite your request, please send a message through the Patient Portal and refrain from leaving a voice message. To send a message to the Referral Coordinator: Log onto the Patient Portal, click messages, then referral request.
